​If you would like to apply for Access & Ability Center services...
​Please follow the "Accessing Our Services"​ steps to begin the process.
​Active AAC student who needs your Accommodations Form? Or needs to check out (or re-check out) your current equipment?

Complete Request Authorized Accommodations (AA) and we will send the signed AA Form to your COS email.​ If rechecking out equipment you already borrowed, submit a new request and check the appropriate items on the form.
​Need to securely submit a form to us ​electronically?

​Use our ​Secure Stud​ent Document Upload System. Be sure to choose "Access & Ability Center" and the type of form you are submitting to us.
