​Current AAC student and you need your Accommodations Form? Or need to re-check out your Spring equipment for Fall?
Complete Request Authorized Accommodations (AA) and we will send the signed AA Form to your COS email.​ If rechecking out equipment you already borrowed from us, submit a new request and check the appropriate items on the form.
