​It is fair to say that even with the best efforts of everyone involved, a problem may occur while providing you with services at AAC. If this should happen, we strongly encourage you to let us know so that we can work together to solve the problem as quickly as possible. Experience has shown us that many times a problem arises because of a misunderstanding or miscommunication; clarification can sometimes be a quick and effective solution. Here you can submit a complaint or comment to our office. This form is designed for students in the AAC program to contact our program concerning potential complaints in efforts for us to reach a mutually beneficial solution to both our students and our program.​

Other COS staff and/or faculty can also su​bmit an AAC Complaint or Comment using this form.

You must be a COS Student/Staff/Faculty to submit a complaint or comment regarding the Access & Ability Center. If you are not affiliated with COS and do not have a Banner ID, please contact our front office at 730 – 3805 so we may assist you further.
