​Welcome! 
We are glad you have decided to make your mental health a priority.  
The mission of our mental health counseling program is to provide students with research-informed, ethically sound and culturally sensitive mental health services. We support students by providing a safe environment for them to enhance their self-awareness, interpersonal skills and personal development. 


If you are seeking services during the FALL or SPRING semesters please download the COS Mental Health Services Welcome Packet below for more information on counseling and what to expect.

The first step is to complete and return the mental health counseling registration forms​ (below) to the Student Health Center on your preferred campus. The forms can be picked up at the Student Health Center or downloaded by clicking on the link below.

COS Health Center Telehealth consent.pdf (only required if you are requesting telehealth or teletherapy services)​

If you are unable to come to campus in person you may submit the forms through the secure portal. Follow the link below: 
Please click this link and follow the instructions on how to return your electronic forms in a confidential manner through our secure portal. NOTE: Each form must be submitted separately.
