 Externship Requirements

Students are required to submit the following information during the beginning of the program to qualify to be placed at externship site. All the following requirements​ are due by October 15th of every year.

A Clear Background & Drug Screen:

Note: 

  • a flagged background will not allow you to be place at an externship site to complete required clinical hours. 
  • with a flagged background, you will need to clear your record (expunged) before taking this course. 

Order your Background:

You will order your Background & Drug Screen through Complio. Please follow the instructions below. ​

Background & Drug Screen Instructions

Once you complete your online order through Complio, you will pick up a Chain of Custody form from John Muir 125. You will take the Chain of Custody form to Quest Diagnostics (any location). Once completed, you will be emailed a copy of the report as well as Nursing & Allied Health office. 

Note: Do NOT drink too much water, it will dilute your urine and you will have to pay additional $45.00 to retest. 

Basic Life Support (BLS Provider) CPR card:

College of the Sequoias Nursing & Allied health program ONLY ACCEPT American Heart Association CPR cards. (Do NOT TAKE American Red Cross, NOT ACCEPTED) 

If you need your card, you have three options:

OR

  • Sign up for an BLS Online CPR ( lecture portion of course is done online, you will still have to do a in-person skills portion at any American Heart Association center. To sign up for your Hands On- Skills Portion

OR 

  • Sign up for COS HLTH 406 CPR for Healthcare Providers (Spring 2024). A $60.00 materials fee is linked to this course. The materials fee can be paid through the Cashier's Office or through your BannerWeb. Must pick up the CPR manual the day before your class. Can pick up manual through Nursing & Allied Health Office, John Muir 125.

Transportation:

A copy of your California Driver's License with Auto Insurance Card (You do not need the car insurance policy under your name, you just need to be on a insurance policy (ex. Under parents' policy) A copy of your auto insurance card with your name on it or a copy of the policy information form with the auto insurance card. ​

OR 

Due to not having a Driver's License and/or auto insurance, for this reason will not be driving, but have reliable transportation to your externship site. Please fill out and sign  a Transportation Agreement and turn it into the Nursing & Allied Health office, John Muir 125.

Transcripts:

A copy of your High School transcripts or copy of your high school diploma. (Official High School Transcripts will be required for your California State Pharmacy Technician registration application). 

OR 

A copy of your GED transcripts or a copy of your GED Certificate.(Official GED Transcripts will be required for your California State Pharmacy Technician registration application). 


Physical: 
A Physical exam must be signed by a Physician or a Nurse Practitioner through a doctor's office.

Note: 

If your doctor office is going to over charge you to complete the Pharmacy Technician​ Physical form, please get a regular physical exam and turn in the physical exam report. 

 


  

Immunizations:

A copy of the following:

  • COVID-19 Vaccine: Either 2 doses of Pfizer or Moderna​ or 1 dose of Johnson and Johnson. Note: Starting 9/2023, two-dose series of a COVID-19 vaccine no longer are manufactured. A COVID-19 Bivalent Pfizer or Moderna booster dose is accepted. Bivalent dose is good for 4-6 months, (You can not mix this vaccine with any other vaccines)
  • Hepatitis B: 3 Doses or Titer (Blood work showing immunity)
  • MMR (Measles, Mumps, And Rubella): 2 doses or Titer (blood work showing immunity) 

Note: with MMR Titer make sure your doctor office test for Measles, Mumps, and Rubella. Many doctor office do not test for measles,) 

  • Varicella (Chickenpox): 2 doses or Titer (blood work showing immunity) 

Note: Having the chickenpox as child does is not acceptable, will have to get a titer to show proof of immunity. 

  • Tdap: 1 dose (Expires every 10 years) 
  • Influenza (Flu shot): 1 dose annually (Student can get a free flu shot through COS Health Center.) 
  • Two Step TB Skin Test (Process must be done twice; Students can get a free Two Step Tb skin test through COS Health Center.)

COS Health Center can complete some immunizations for students for a small fee. Minus Tb skins test and flu shot are free for students. Please contact the Health Center at 559-737-6195 for more information regarding immunization hours and immunization fees. ​
