About two weeks after you've filed your FAFSA the Financial Aid office will contact you to complete your file. Once all required documents have been completed your file will be processed to be awarded.  ONLY COMPLETED FILES WILL BE AWARDED.

Printable and Fillable Forms

Below you can find a list of printable forms of our most common Financial Aid documents. Most of the forms are also PDF fillable which means you can type in your information on the form, print, and sign.  To check your status and which forms are needed to complete your Financial Aid file log on to "Banner Web".  Please submit completed forms the Financial Aid office, Sequoia Room 105. 

Please only submit forms that we are requesting.
2011-2012 forms are for semesters Fall 2011, Spring and Summer 2012. 
2012-2013 forms are for semesters Fall 2012, Spring and Summer 2013.
